We changed web servers on 6/14/05 and the following described problems may no longer be an issue: Some classmates have reported problems submitting this form through the use of the "Submit" button at the bottom of the form. The problems appear to be limited to those not having a mail program configured on their PC and/or are using AOL or other web-based email program. If you do not receive a confirmation page after clicking on "Submit," a workaround solution is to fill in the form and then copy and paste it into the body of a new email document you have addressed to us:
Use "TAB" key or "MOUSE" to move from one field to the next. Press the "Submit Form" key located at the bottom when finished.
CAUTION - Avoid pressing the "ENTER" key, as it will immediately submit only data entered prior to pressing the "ENTER" key.
IMPORTANT: New or additional information may not be processed unless (1) a valid email address is supplied or (2) the field "NAME OF PERSON SUBMITTING THIS UPDATE (if not the classmate listed above)" is filled in with the classmate's name entering and transmitting this information.
PLEASE USE ALL CAPS and observe the requested formatting (i.e last, first middle <SMITH, JOHN WAYNE>)
